Varenicline (Chantix)
Selective alpha4-beta2 nicotinic acetylcholine receptor partial agonist approved for smoking cessation. Reduces cravings and withdrawal while blunting the reinforcing effect of nicotine. Standard titration: 0.5 mg daily x3d, 0.5 mg BID x4d, then 1 mg BID. Plasma half-life ~24h. Oral bioavailability high; predominantly renally excreted unchanged. Neuropsychiatric warnings historically present; 2016 EAGLES trial largely reassured.
