Fluvoxamine (Luvox)
SSRI with uniquely potent sigma-1 receptor agonism, the only SSRI with clinically relevant sigma activity. First-line for OCD (FDA-approved). Strong CYP1A2 and moderate CYP2C19 inhibitor; significant drug interactions (contraindicated with tizanidine, theophylline). No active metabolites. Nonlinear pharmacokinetics at higher doses. Also studied for COVID-19 outcomes (sigma-1 mediated anti-inflammatory effect via IRE1/XBP1s pathway).
