Paliperidone (Invega)
Atypical antipsychotic D2/5-HT2A antagonist; it is the 9-hydroxy active metabolite of risperidone. Available as oral ER tablets (OROS) 3-12 mg QD and as long-acting injectables (palmitate monthly, three-monthly, and six-monthly). Plasma half-life ~23h for oral ER. Oral bioavailability ~28%. Minimal CYP metabolism, predominantly renally excreted (~59% unchanged), fewer CYP drug interactions than risperidone. Hyperprolactinemia and orthostatic hypotension are prominent AEs.
