Reduces HbA1c, blood pressure (osmotic diuresis), and body weight (~2-3 kg). Landmark EMPA-REG trial showed 38% reduction in cardiovascular death — now first-line for heart failure with or without diabetes. Also approved for CKD. UTI and genital mycotic infection risk. Rare DKA risk even with normal glucose.
