Nucleotide reverse transcriptase inhibitor (NRTI) prodrug; rapidly hydrolyzed in plasma to tenofovir (TFV), which is then phosphorylated intracellularly to tenofovir diphosphate (TFV-DP), the active moiety. Used for HIV-1 treatment and as PrEP (with FTC as Truvada). Plasma tenofovir half-life ~17h; intracellular TFV-DP half-life ~50h in PBMCs, enabling once-daily dosing. Oral bioavailability ~25%. Associated with nephrotoxicity and bone mineral density loss with long-term use.
