Baclofen (Lioresal)
GABA-B receptor agonist used for spasticity from spinal cord injury, MS, and cerebral palsy. Also used off-label for alcohol use disorder (reduces craving and withdrawal). Crosses blood-brain barrier poorly (hence high doses needed orally), intrathecal pump available for refractory spasticity. ~80% oral bioavailability, predominantly renally eliminated unchanged. Abrupt discontinuation risk: withdrawal can cause seizures, hallucinations, and autonomic instability.
